I/We hereby voluntarily consent to share Aadhaar details and OTP for the purpose of identity verification, account opening, and related banking services. I/We understand that the Aadhaar information available with UIDAI, including personal details, permanent address, communication address, and other demographic information, may be used by The South Indian Bank for validation and processing of the account opening request. I/We confirm that the Bank officials have explained the nomination facilities and applicable rules, including provisions relating to nomination, successive/ simultaneous nomination, and other applicable statutory provisions. I/We understand the benefits and implications of registering or not registering a nomination in the account. In case nomination is not opted for, I/We confirm that the benefits of nomination and the consequences of not availing the nomination facility have been clearly explained by the Bank officials. For accounts opened in the name of a minor, I/We understand that guardian consent and Aadhaar-based verification of the guardian are mandatory. I/We confirm that the guardian details furnished are true and correct and that the guardian is the natural/legal guardian of the minor applicant. I/We confirm that no false, misleading, or incorrect information has been provided in relation to the minor or guardian details during account opening. I/We undertake that, upon the minor attaining the age of majority (18 years or 21 years in case of court appointed guardian), status of the account holder shall be converted to major and KYC updation shall be completed within one month from the date of attaining majority. Continued operation of the account shall be subject to successful completion of the required KYC updation and related formalities. I/We understand that the SIB Campus Account is intended primarily for savings, educational, and school/college-related transactions. The Bank may impose restrictions on opening deposits or other account types linked to this product, except as specifically permitted by the Bank. Staff and students hereby give consent to use their mobile number and email id for generation of OTP by The South Indian Bank and confirm that I/We am/are aware of the terms and conditions applicable for the scheme of SIB Campus.
Institutional users and authorized staff provided access to the portal hereby undertake to maintain strict confidentiality of all student/customer information accessed through the portal. Institutional users shall not share, disclose, misuse, transfer, or provide student/customer data to any unauthorized third party or use such information for personal gain or non-authorized purposes. Portal login credentials issued to authorized institutional users are confidential and shall not be shared with any other person. The institution shall be responsible for ensuring secure handling of login credentials and portal access. All information and data accessed through the institutional portal shall be treated as confidential and handled in accordance with applicable data privacy, confidentiality, and information security requirements. The South Indian Bank shall not be liable for any loss, damage, unauthorized access, misuse of information, or consequences arising due to sharing of login credentials, mishandling of student/customer data, or breach of confidentiality by the institution or its authorized users. I/We further affirm that the bank shall not be responsible for any failure or malfunction of digital products and services, due to the circumstances beyond its control. The bank shall not be responsible for any dispute consequential or indirect, loss or damage arising from or related to the use of User ID, password, PIN, OTP of digital products and services. The Institution shall be solely responsible for all the transactions affected by the use of User ID, OTP and Password of digital products and services. The Institution hereby agrees to indemnify and keep indemnified the Bank, its officers, employees and representatives from and against any and all losses, damages, claims, liabilities, costs, charges and expenses whatsoever that may arise on account of any unauthorised, fraudulent, improper or misuse of the User ID, password, OTP, DSC authentication credentials or any other access mechanism provided for operating the module and the Bank shall not be liable for any loss or damage suffered by the users/ Institution on account of transactions carried out using such credentials.
ACCOUNT OPENING RULES: All the necessary documentation as mandated by the Regulatory / Bank authorities should be provided for opening the account. • All accounts should maintain the stipulated average monthly balance based on the product program and branch in which the account is opened. • Savings accounts can be opened only by individuals for non-business purposes. • In case of any complaint relating to features of any of the product, the Grievance Redressal Cell within the bank can be approached for a resolution at customercare@sib.bank.in and if not resolved satisfactorily within 30 days the same may be escalated to Reserve Bank of India via https://cms.rbi.org.in or sent physically to Centralized Receipt & Processing Centre, Reserve Bank of India, 4th Floor, Central Vista, Sector 17, Chandigarh - 160017. The bank reserves the right to close the account in case the savings account is used for business purposes as evidenced by the transaction behavior. • Adequate balance should be maintained in the account before issuing cheques. • Details of charges of funds transfer, inter branch banking and other services are available in our website - "Service charges" & Fees Brochure and the Code of Bank's Commitment for individual Customers can be obtained from the website. Interest on savings account will be paid at the rate stipulated by RBI from time to time. • No unarranged overdraft would be allowed in the Savings Account. In case of exceptions, the bank would charge interest at commercial rate. The bank reserves the right to close the account in case of unsatisfactory conduct of the account. • In the event of death of one of the joint account holders, the settlement of the account shall be effected in accordance with the mode of operation mandate given to the Bank. I / We agree to promptly notify the Bank of any change in permanent / communication address / contact details provided under my / our CIF ID with supporting documents as mandated by the rules of the Bank. I / We also agree to submit my/ our KYC documents as and when my accounts are due for KYC update. I / We will not hold the Bank responsible for any disruption in account operation resulting from non-submission of OVD, as mandated. I/We hereby agree to share my/our personal information (including mobile number & email) to the external vendor for verification purpose. In case number or volume of the transactions in the account are found to be not matching with the profile declared by the customer, Bank has the right to put restrictions in the operations of the account and customer should provide satisfactory clarifications for the reason for such high number or volume of transactions for getting the account unfrozen. In view of the fast pace of digitalization, and as an adequate safeguard, customers are advised to update their Mobile number and Email Id, with bank, so as to receive regular updates regarding their accounts and transactions. Customers are advised to contact home branch / bank's Toll-Free number, through the contact numbers published in bank's website, in case of instance of non-receipt of alerts, through SMS or Email. In the best interest of customers, bank advises installation and regular/periodic use of digital channels such as Mirror+ (Mobile banking) and SIBerNet (Internet banking), to be up to date, and fully aware of all accounts and transactions therein. This will additionally help customers in bringing to the notice of the bank, any instances of unauthorized transactions in their accounts.

I submit my Aadhaar number/ physical Aadhaar card /E-Aadhaar card /Masked Aadhaar /Offline Electronic Aadhaar xml and voluntarily give my consent/ no objection to: Use my Aadhaar details for authentication/ offline verification and fetching my details from UIDAI through bank's authentication/ verification systems. Use my mobile number mentioned below for sending alerts to me. Link my Aadhaar number to all my existing/ new/ future accounts and Customer Profile (CIF) with your bank and store the details for the purpose mentioned in 4 below. 3. I am informed by the Bank, that submission of Aadhaar is not mandatory, and there are alternative options for KYC and establishing identity including by way of physical KYC with officially valid documents other than Aadhaar. For e-KYC/authentication/offline verification, Bank will share Aadhaar number and/or biometrics with CIDR/UIDAI, and CIDR/UIDAI will share with Bank, authentication data, Aadhaar data, demographic details, registered mobile number, identity information, which shall be used for the informed purposes mentioned in 4 below. 4. I authorise and give my consent to the Bank (and its service providers), for following informed purposes: Account opening/ Customer ID creation/service requests etc. and periodic KYC process as per the PML Act, 2002 and rules thereunder and RBI guidelines, or for establishing my identity, carrying out my identification, offline verification or e-KYC or Yes/No authentication, demographic or other authentication/verification/identification as may be permitted as per applicable law, for all accounts, facilities, services and relationships of/through the Bank/, existing and future. Collecting, sharing, storing, preserving Information, maintaining records and using the Information and authentication/verification/identification records: (a) for the informed purposes above, (b) as well as for regulatory and legal reporting and filings and/or (c) where required under applicable law; producing records and logs of the consent, Information or of authentication, identification, verification etc. for evidentiary purposes including before a court of law, any authority or in arbitration. 5. I understand that the Aadhaar number and core biometrics will not be stored/ shared except as per law. I have downloaded the e-Aadhaar myself using the OTP received on my Aadhaar registered mobile number. 6. I will not hold the Bank or its officials responsible in the event this document is not found to be in order or in case of any incorrect information provided by me. The above consent and purpose of collecting Information has been explained to me in my local language.
For minor, I confirm that, I am the lawful parent/legal guardian of the minor Aadhaar holder. I authorise the use of my Aadhaar details for authentication/ offline verification from UIDAI. I understand that the Aadhaar details of both me and the minor will be shared with the bank and UIDAI for identity verification.

I/We declare that I/We am/are aware of the advantages of nomination/benefits of nomination were explained to me/us. I/We hereby confirm the receipt of MTC, CKYC in my name can be used by the bank for fetching KYC details as well as for periodic KYC updation. I/We undertake/authorize South Indian Bank that all information provided by me of any nature including personal & sensitive information relating to account/investment/credit facility can be shared with/to other South Indian Bank Branches, its group companies including any affiliates and subsidiaries/banking financial institutions/credit bureaus/agencies/service providers who have an agreement with South Indian Bank. If I intend to revoke my consent to the sharing of the data, the products/services available to me, pursuant to the consent provided earlier, shall no longer be available to me, and I shall be required to initiate closure of such products/services. I/We have understood that non-maintenance of the Average Monthly Balance will attract charges and the same have been explained to me. I/We understand the detailed charging structure for non-maintenance of minimum average monthly balance, which is available on bank's website www.southindianbank.bank.in. If there is no balance in the account to satisfy the service charges, I will provide sufficient balance in the account and bank may mark a lien on the subsequent credits in the account and recover my dues. I hereby certify that the Savings Bank Account would be used by me to route transactions of only non-business/non-commercial nature. In the event of occurrence of such transactions or any such transactions that may be construed as commercial/ business/ dubious or undesirable, the Bank reserves the right to unilaterally freeze operations in such account and/or close the account. I/We also understand that in case of liquidation of the Bank, DICGC is liable to pay each depositor through the liquidator, the amount of the deposit up to Rupees Five lakh within two months from the date of claim from the liquidator. I/We also accept all charges that are not mentioned here, but corresponds to the product chosen by me. I/We also understands that charges that are mentioned in MTC and confirms that I/We have received a copy of the same and will not hold bank responsible on penalizing me/us on defaulting on the conditions laid down and updated by South Indian Bank Limited www.southindianbank.bank.in. I/We, the undersigned, have read, understood and agree to absolutely and unconditionally abide by and be bound by the Terms and Conditions displayed on website www.southindianbank.bank.in as revised from time to time by South Indian Bank Limited, in relation to all of my/our accounts, for present and future, maintained/opened/to be opened with South Indian Bank Limited. I/We confirm that the product features of account have been explained to me. I/ We hereby acknowledge receipt of the regulations of Savings. I/We hereby provide the consent to send information/ promotional messages through SMS, Whatsapp, Email & RCS. (In this context, "I", "my" and "me" refers to all holders of the account) have read and understood the T&C and understand that any changes to the T&C will be available on the website www.southindianbank.bank.in only. I/We hereby provide the consent to download my KYC Records from the Central KYC Registry (CKYCR), only for the purpose of verification of my identity and address from the database of CKYCR Registry. I/We understand that my KYC Record includes all my KYC Records/Personal Information including my name, address, date of birth, PAN number etc. In the case of Minor account: Guardian declares that the withdrawals from the account will be made only for utilizing the amount for the benefit of the minor. Guardian shall indemnify the bank against the claim of the above minor/s for any transaction/withdrawal made by me in his/her account.
